Since anyone who comes to the U.S. usually comes to New York City, I thought I'd start a thread of restaurants I like (and that have good ratings.)
This week-end my son took me to BEC in Chelsea (Bacon, Egg, and Cheese). I was hesitant because although I love a good bacon, lettuce and tomato sandwich, I'm not crazy about eggs.
Well, I'm a convert. These sandwiches, made with bread they bake every day and with all organic products, are just delicious.
If you're in the area, give it a try.

I'd also really recommend going to Katz' Deli for the quintessential New York Deli experience. The prices are enough to raise your blood pressure, but it's worth it in my opinion. Of course, I love deli meats, so bear that in mind.
I don't know if sandwiches like this are available in Europe, but I certainly never encountered anything like them in Italy.
It would be a toss up for me whether to go for the hot corned beef, hot pastrami or hot reuben sandwich, but maybe the reuben edges the others out. Also, order the potato pancakes; I love them.

Be advised it's an elbow's out quintessentially New York kind of place. Go elsewhere if you want somewhere more "civilized".
