I have such data for Poland:
According to Polish economists during the period 1000-1600 Poland's GDP per capita was 75% - 90% of Western Europe's GDP per capita.* But by 1700 it declined to 55% and by 1790 it declined to 46.5%.** So the gap emerged largely during the 17th century and the 18th century.
*Here are the values for particular years:
1000 - 85.5%
1250 - 77.3%
1370 - 88.5%
1470 - 77.6%
1580 - 80.5%
1600 - 88.0%
**Actually it declined to 35.8% in 1720 and then between 1720 and 1790 it bounced back to 46.5%, but this was still lower than the 1700 level.
